Looking for a cpt for a left femoral vein angiogram done during R/L heart cath with coronary angiography. Can anyone help?
 
If the full RHC was performed (no pressures were documented), the you can bill 93460 and 93505 for the biopsy. The venogram was for sheath placement and is part of the exam.

The pressures are documented, I just didn't screenshot them due to PHI. Can you shed any light on why we are getting denials on all 93460 / 93505 claims? Insurance has even paid and them come back and reclaimed their money.
