A lot of our doctors reports state "Left heart catheterization without left ventriculography". This is the one that confused me the most. What do I code? Can I still code 93452?
 
you would only use 93454 if your Dr did not do the LHC look at the description of 93458 in your CPT book it states (including LV WHEN PERFORMED) if it is not done you would still use 93458
