If the site of the lesion is not under lesion, do you look under neoplasm by site benign or neoplasm by skin of site?
 
Assuming you have a path report that says benign, then is it just benign neoplasm of skin. If you do not have a path report then you code disorder of skin 709.9 or 709.8.
