Wiki lesion removal by shave & destruction

My physician will shave off a lesion and then destruct the base. When billing these lesions, I am questioning myself. Should it be a destruction vs shave. He also does the same thing for warts. Please help me code these lesions when using combination techniques.
 
For shave excisions, wound cauterization is included in the procedure. Billing for the shave excision would be appropriate.
