There is a lesion on the face by the temple; do not know if it is malignant. it is 2.5 cm . How would you code this and would you use a closure with it?

Look under lesion then skin you will the L98.9 code. That is all you can use until path report is returned. You will not code the closure unless it is intermediate or complex. And documentation must state whether it is a full thickness (excision) or partial thickness (shave) removal. An excision you must hold the claim and wait for the path report.

For the CPT code you need to know if the procedure was a shave or an excision. If it is a shave then look under shaving. For an excision you will need to wait for path as they can only be coded benign or malignant.

You would need a path report indicating benign, and the procedure note would need to indicated procedure was a full thickness removal.

How would you code this in cpt for a 2.5 cm lesion.

Again there is not enough information. You must know the depth of the removal and if it was an excision you will need a path report before it can be coded.
