Hi,
How to bill Leuco if it's given with an inital chemo drug or a sequential chemo drug.

Oxaliplatin 10-12 96413
Leucovorin 10-12 ?

Avastin 96413
Decadron 96367
Oxaliplatin 96415
Lecovorin ??

I was told that if Leuco is given with a sequential chemo it should be coded as a sequential 96367 and + one additional 96366. if it's witht he initial chemo drug (96413) it would then be a concurrent 96368. HELP
 
For the scenario you have provided I would code a 96413 & 96415 for the Oxaliplatin and a 96368 for the Leucovorin. Because the Leucovorin was run concurrently, you would code as such, but you can only bill for 1 concurrent hour of infusion. I hope this helps.
