Can someone assist on locating different levels of supervision requirements; when can non-licensed personnel bill for services w/ no licensed provider on site? Specifically looking for guidelines on billing 99211 and/or 36415 (venipuncture).
 
In a physician office setting, if the person providing the service has no license or an NPI number then a provider (MD, NP) must be on site while the patient is being seen. Even a licensed RN or LPN must have direct physician supervision in the physician office setting. the rovider you are using the Npi for in field 24J must be physician present in the office while the patient is being attended. What services are you billing for when there is no provider present?
