I need clarification for 93454 and 93458. 93454 is catheter placement for angiography only and 93458 is used when angiography and ventriculography is done? Is that correct??
 
93454 is angiogram only and does not include left heart catheterization.
93458 includes angiogram and left heart catheterization, with or without left ventriculography.
 
93454 is when a catheter is placed in the coronary artery and an angiogram is done.

93458 included the work of 93454 (selective coronary angiogram) and additional work of crossing the aortic valve and intracardiac and intravascular pressures are recorded. You may also do a left ventriculography at this time, but is not required although almost always done.
