Hello fellow coders. I am general surgery code for a colorectal surgeon who does LIFT procedures. We have been using code 46275 (Surgical treatment of anal fistula (fistulectomy/fistulotomy); intersphincteric) and our provider is stating that because these are more extensive than a fistulotomy/fistulectomy, can we append a 22 modifier if the documentation supports this? Or is there any other codes that will support this type of procedure?

I think this highly depends on what exactly the provider means by 'these are more extensive than a fistulotomy/fistulectomy.' Here is a direct link to an AAPC article regarding modifier 22 and the instances in which would be supported. AAPC Modifier 22
