Are there any specific guidelines for reporting LMP? What if the patient doesn't remember her LMP? What if it's irregular and it was months ago but she's only 10 weeks?

Anytime code annual paps or any GYN processes or treatment asked the female patient her LMP. The provider usually documents it. If in notation for the day use it by noting on claim. If not documented, then leave blank ...you can only go by documentation. We get denials if not noted per some payers.

If you are indicating use of LMP for pregnancy documentation, you would leave it blank it the provider indicates the patient does not know. The pregnancy will be dated via ultrasound, not LMP as it is more accurate than LMP in the first trimester. If you are referring to a gyn patient, you would have to mark it as unknown with a note of explanation in the body of the note for the visit.
