LMSW providing consultation

Does anyone know what code to use when an LMSW is providing consultation to a primary care provider? This is a 15-30 minute consultation in the exam room or office and the focus is on functional assessment and behavioral recomendations to the patient and provider. Thanks for any help and guidance!
 
With the info provided sounds like the Health and Behavior Assessment/Intervention codes might work - Take a look at 96150/96151
