For RFA - Radiofrequency Ablation ( 64633 - 64636 ), do I need to code Local anesthesia seperately?

and for joint and bursa injection - 20610 , 20605 do I code local anesthesia seperately , 3 ml of 1% Lidocaine. ?? And Provider uses fluroscopy so do I code
for ex : left shoulder joint injection - 12 mg betamethasone
20610 - LT
77002 - 59

Local is not separately reportable for any procedure. Its inclusive which is why there is no code. If you have access to a source with the full RVU breakdown it will appear under supplies under the Practice expense.


Its addressed in general surgery guidelines in the CPT book. Second bullet under CPT Surgical PackageDefinition lists as part if inclusives:
Local infiltration, metacarpal/metatarsal/digital blockor topical anesthesia

Look it up yourself. The answer is not a secret. use your CPT book for the the answer is right there by the 77002. Also you should not be putting 59 with add-on codes.

But in 2017 edits 77002 is an add on code but physicians performing this service so it should be
