Can we hire a locum tenens for our ob doctor who is on vacation and he still read his nst's when he returns and then we billed under him without the q6 modifier?

Not sure I am following the question, when you say "he still read his NST when he returns". Are you saying:
* Your OB is on vacation
* When he returns to the office he will read his own NST's
* You will bill under the OB for the reading of the NST
Why would you hire a Locum if the OB is going to read the NST himself? What service would the Locum be rendering?

The OB provider is on vacation and he would read his own NST's. We would bill those NST's under him. But the locum is here for any emergency deliveries, OB patients that come into the ER. So we would bill the deliveries, triage, and NST's done by the locum with the Q6. So there would be NST's for the same DOS with and without the Q6 modifier.
