I'm looking for opinions on which anesthesia code you would select for the following procedure:

POSTOPERATIVE DIAGNOSES:
1. Thrombosed arteriovenous fistula of the right upper extremity.
2. Thrombosis of the right brachial artery.
3. End-stage renal disease.
4. Thrombus of the right subclavian vein.

OPERATION PERFORMED:
1. Thrombectomy and revision of the arteriovenous fistula of the right upper
extremity.
2. Venogram of the right upper extremity and superior vena cava, angiogram of
the arteriovenous fistula of
the right upper extremity, arteriogram of the right upper extremity.
3. Right brachial and radial artery thrombectomy.
4. Right subclavian vein thrombectomy.
5. Balloon angioplasty of the right cephalic vein and the brachial artery.

I don't have a list of the base unit values from CMS, but out of these procedure codes here I would use the one with the highest base unit value:
01442, 01840, 00350
