I hope someone will be able to help me.
Lately surgeon's are documenting:
"There were diffuse grade 2/3 articular
cartilage changes from the medial femoral condyle. There was a
radial tear involving the posterior horn and the medial meniscus
near the root attachment along the medial aspect of the medial
tibial plateau, under the medial meniscus body, there were 3
osteochondral loose bodies"

Coding as 29999, is this correct?

Based on your description of where the osteochondral loose bodies were found (and I presume removed from there and the joint), they were in fact intra-articular, and the procedure would include 29874: removal of loose body(ies) if it was a distinct/separate part of the procedure, and not something that would have been a part of or included in some other arthroscopic procedure done at the same time that might by NCCI edits include removal of loose bodies.

I hope this isn't too confusing.

Respectfully submitted, Alan Pechacek, M.D.
