Name of operation-LT knee arthroscopy with arthroscopic chondroplasty patella
post/op DX- Grade 3 chondromalacia of patella

It was coded as 29877-LT/717.7 and 733.92

A 30 degree scope, Dandy nerve hook, basket forceps and articular shaver were used. Knee was examined in a systematic fashion with the following findings: Medial compartment, medial gutter was all right. Medial meniscus showed no evidence of tears. Mediofemoral condyle, medial tibial plateau also looked pristine. Intercondylar notch showed posterior cruciate and anterior were intact. Lateral femoral condyle and lateral tibial plateau are all right. Lateral meniscus showed no abnormalities. In the suprapatellar pouch, there is a moderate amount of debris floating in the joint, small white particles and these appear to be coming from the medial facet of the patella where there was extensive grade 3 chondromalacia with very limited grade 4 chrondromalacia. Loose flaps of carti;age were shaved with an articular shaver with the scope in the??____ superior lateral portal to remove all loose flaps of articular cartilage. Minimal areas of exposed bone were indentified. The lateral facet showed no abnormalities. The medial tibial plateau approximately showed 1 small area of chondromalacia, but the trochlear groove was very shallow and showed no evidence of damage to the articular cartilage.

Our clinic is under the impression that if 29877 is the ONLY code billed, it is appropriate to use it. If it is done in conjunction with another procedure, then the G0289 rule comes in to play. G0289 is an add-on code, not a stand-alone code. :)
