Is there anyone that is familiar with SNF dx coding ranking for a skilled patient that goes to the hospital and comes back during their skilled stay?
 
Thank you for responding.

Scenario: Patient is skilled with Prim DX of : AFTERCARE FOLLOWING EXPLANTATION OF HIP JOINT PROSTHESIS.

Breaks her hip while she is in our care goes back to the hospital and has a total hip replacement.

How should this be coded when she returns to our facility? I am thinking the primary aftercare code comes off and is replaced???
