When the report says :

"Left Ventricular pressures were recorded, however left ventricular angiography was not performed. There was no gradient across the aortic valve on pullback."


Was an LV done? We have 4 coders in our office and two say yes and two say no. The pressures are dictated in the report.

As long as LVEDP were performed (and it appears that they were) then you have a true left heart cath, 93458. Sometimes LV's are not done bc the patient has some renal issues.
