Non-Hodgkin lymphoma in remission documented in assessment. No MEAT found in the medical report. Can we code the current code or code it as history?

Thanks in advance for any help/advice.
 
Lymphoma is a systemic disease and is never coded to history UNLESS specifically noted as such by the physician. Codes are assigned based on the current/active/last known site of disease. Hope this helps.
 
Since this query is regarding risk adjustment coding, can we code directly from the assessment because no evidence of current/active/last known site of disease is mentioned in the medical report neither any medication is listed in the current medication, without any evidence can we consider it as active just on the basis that it is mentioned in the assessment?
 
Sorry, didn't notice that this was in Risk Adjustment. I don't know the rules around that coding so I'll defer to someone with that particular experience.
