M79.66 doesn't have a bilateral code. They have right, left and unspecified. How would you bill bilateral. I am just using one for left and one for right. It doesn't make sense to not have one, unless i'm missing something.
 
Leg Pain

There definitely isn't a bilateral code and since the pain IS specified as far as the location with the mention of bilateral, I have and would use the two separate codes one for the left and one for the right.
 
M79.66 requires a 6th digit:
M79.661 Pain in right lower leg
M79.662 Pain in left lower leg

If the patient has bilateral lower leg pain, you'd use both of those codes to indicate left and right.
