Can anyone who works at a major carrier (blues, aetna, humana, etc) tell me how you handle a pain doc billing a facility fee for POS 49? Do you pay the surgeon regardless of POS, and then allow a facility fee regardless of POS; to compensate what should be the higher reimbursement of POS 11 or 49?
