I have a chart with Major Depression in History as (MDM) and Dr Diagnosed Depression . Do I code just Depression 311 ( prior to 10-15) or can I code the major depression? On one DOS the HPI documented that the patient had Suicidal thoughts. If you feel I can support the major depression that is in History please provide any documentation you are aware of. Thanks!
