Can someone give me the CPT code for the following.

Dx: malfunction of A-V graft


CPT: fistulogram, angioplasty and ligation of graft.

I'm thinking I need more information such as the op report?

I'm coding for anesthesia and we dont normally get the op report unless we request it. This is a medicaid patient so I need the CPT.
