Wiki Malig.Punch Bx needing re-exc.-Please help

My doc did a punch bx on a pt.- I think back area.Path: Non invasive lentigo maligna melanoma
My question is - Punch bx showed malignancy and this
area needed a re-excision. When the re-excision was done
the path. came back benign. Do I code from the malig.lesion section or not. Path: Skin from rt upper back
exhibiting 1) intradermal nevus;benign 2)
Foci of chronic inflammation Thanks for your help
