Thanks. Have read also that hx of brst ca exam is lately decided by physician. So, my next question is: Rad dept routinely does exagg axilla view on hx of breast cancer pt reportedly to accomodate radiologist, thus with diag and extra view, exam is considered diag. Have read screening can consist of more that mlo and cc if needed and am certain exagg views are done sometimes for routine screenning Can hx of breast cancer with these paticular views ever be billable as screening if original order is screening or does V10.3 diag make anything other than standard views a diagnostic exam?

If the order is screening, then doing the extra views just for protocol does not make it a diagnostic exam.
As long ago as 1996 AMA said that screening mammogram (then coded 76092) is usually 2 views - but that if supplementary views such as Cleopatra and axillary views are necessary they are included in the screening service.
If those screening views are interpreted and based on the findings you then do diagnostic views, you would code both a screening and a diagnostic (GG modifier on the diagnostic for Medicare).
