Wiki manipulation knee (iv sedation)please help

Most of the surgeons in that i biil for do manipulation of knee under IV sedation or MAC but the code states to be done under general sedation(27570) so i bill 27599 (unlisted code) which medicare will not pay for 27599 code for an ASC. Does any one have any suggestion?
