Has anyone had luck with payment from the insurance carriers for manipulations under anesthesia. Has anyone use modifiers 22/23 for field avoidance, or when multiple sites are being manipulated, if the doc is numbering them does the order of manipulation come into play. #1 must be used first? I believe the highest ASA get the billing, I just want to confirm.

And does it really matter if there is any field avoidance? What about the 2nd and 3rd procedure, just bill them as the first procedure? We have received payments back clearly inconsistent between the carriers and our test cases. Not sure if they will want the money back or we really get to keep it :-)
