Can anyone give me some guidance to the following situation. I have a case where a conglomeration of melanomas were resected , one massive resection of 30 cm from the subcutaneous tissue of the chest. based on the size I would look to 21558 - but there is a foot note that states - For radical resection of tumor(s) of cutaneous origin [eg.melanoma] see 11600-11620 - this would lead me to use 11606 for an excision of 4cm or more??
is that really correct?
is there ever a time when a higher level code would be used for an excision of Melanoma?
what if it were involving the fascia?
