Good morning all,

I am trying to create a 'master list' of the lenses and the HCPCS codes that they are assigned. At this time, I have approximately 90 different lenses, most with the applicable HCPCS code assigned to it. I am missing a few though and need help either locating the HCPCS code or if you know, that would be helpful also.

HOYA: YA-60BB; VA-60BB and PY-60MV
STARR: AQ2003V; AQ2010V; AQ5010V; AA4204VF; AA4204VL
