Could you please advise on the proper use of maternal care codes?

Are maternal care codes such as O34.73 (Maternal care for abnormality of vulva and perineum, third trimester) only used when the abnormality creates a issue? Or should O34.73 be used when the abnormality exists, even if it doesn't impact the outcome?
