MDM-number of diagnoses/management options

Hello,

Any help would be greatly appreciated. If you have a new patient visit would you consider under number of diagnoses/management options all the diagnoses as "new problems" even if they have a specific diagnoses such as anxiety? I was taught from the auditor at my work that if the patient is new then the diagnoses would always be new in that area. It did not say this when I did my CPMA course. It is also not in the E/M guidelines.
