When talking to a provider about MDM under the table of risk I was always told it is how the patient is right now not what could happen to the patient. Is there anywhere I can find this in writing? Thank you
 
Do you know what page? I must be missing it. I have a provider who wants to bill new office visit 99025 for gastric bypass patients.
