Wiki Measurements for Lesion Excision

I might be overthinking this, but if a doctor says that a lesion is .5cm in size, and that is was excised using a 1cm elliptical excision, what would you use for the total excised diameter for CPT selection?

Does this indicate the margins were .25cm?

I am new to skin lesions. How do I calculate the skin lesion if its stated as :

The primary site is removed with 1 cm measured margins, 2.2 x 4.5 cm.

I know its in the 11640-11646 range

Help is much appreciated :)

The way the note is written 2.2 x 4.5 cm includes the 1cm margins. So 4.5cm is the measurement

Trying to figure which code to use for size. 11400x series

Right Areola Nevus skin lesion-- 1.1 x 0.5 x 0.2 cm Benign.

Sternum skin lesion---1.3x 0.7 x 0.3 cm Benign
