I am still kind of new to this and need some guidance -- If a note says -- periwound measures 6x9cm with 2 areas inside measuring 0.7X1 and 0.5x1.1 -- which is the actual measurment to be used for coding the 1 big area or the 2 little areas -- also if the note says that the ulcer has pseudoeschar is that still considered an open ulcer or a closed ulcer? Any information would be appreciated. Thans
