A psychiatrist who performs med management checks at nursing homes recently went to a coding conference where she was told by a consultant to use subsequent nursing home care codes (mainly 99309) instead of the med management code 90862 mainly b/c reimbursement was higher. Does anyone else do this or have an opinion on it?
 
I would say choose the code most appropriate for what was done not based on reimbursement. Either a 90862 or one of the subsequent nursing facility care codes (99307-99310).

90862 is used specifically for psychopharmacologic management, E/M codes are more general & may include review of comorbid medical conditions.
