I coded the following OP Notes with 39000 and 11044-51:

The Surgeon disagrees- He feels it should be 21600 and NOT 39000.

The OP Notes Read:

The inferior aspect of the mediastinotomy wound on the right side was incised. We noticed there was a sinus tract, which was excised completely en bloc creating an egg-shaped defect. We noticed that the sinus tract connected to the edge of the rib, which required excision with bone cautery. Approximately 1 inch of the bone was excised. Once we had debrided to the sinus tract we proceeded to use the curet instrument to further debride the area.

Who is right? Me or Him?
HELP!!!
 
I don't think either one of you are wrong, but i would have to agree the 21600 would be the best code for the information you have given. According to the coders desk reference i have 21600 includes debridement and irrigation around the area.
