Wiki medicaid and mod 25

I work in a clinic that sees mostly medicaid pts. When we administer vaccines with a well visit I put mod 25 on the visit. Medicaid denies the mod 25 and doesnt pay the administration fees only the injections. Should I only bill for visits and injections minus the administration fees and mod 25?

Yes I add a -25 mod for e/m with -SL on the vaccines and administration.
