Can a physician bill for an office visit (99213) and a medical conference code such as 99366 if there is a meeting with the patient and case manager? This would be double billing I think. I would think they could be 99367 in addition to 99213 only if the patient is not present. There is not a code for the physician to bill an extra code if the patient and case manager at the doctor's visit, right?
