Wiki Medical provider needs coding/billing

I am a NP and just starting a practice. EMR company will do billing. Until patient numbers are high enough, I am only employee (no office manager).
1. would you recommend having a coder on-site?
2. do billers handle the denials, or would that be someone in our office?
3. can you direct me where I can learn how to set our office fees?
4. Any other thoughts you care to share???

As a former office manager for several clinics; I would:

1) Hire a hourly remote coder/biller to scrub your claims for the clearinghouse. Someone with experience in both so that they could handle any denials for you. EMR company will not do that.
2) Your rates should be based on your specialty. Roughly twice /thrice what your Medicaid will reimburse is a good start.
3) Are you credentialed yet to bill insurances? If not that will be your first step.
