Wiki Medical Record Completion (Physician)

Does someone have a policy and procedure that you are willing to share that states the timeframe required for physicians to complete their medical records?

I thought there was something in the condition of participations but I cannot find it...or I am definately having a duh moment.

I think I saw that it has to be a month to three month time frame but I am not 100 percent sure.
