Hello,

Can anyone tell me if the teaching physician's bill/note must reflect the "personal work of obtaining a history" since the new change regarding the student documentation?

"CMS recently made a change in the requirements regarding how a teaching physician can use student documentation for a billable service. The change requires that the teaching physician must verify in the medical record all student documentation or findings, including history, physical exam, and/or medical decision making and must personally perform or re-perform the exam and medical decision making but does not need to re-document this work."

Does this apply for both Student and Resident?

Attending Linking statement to student notes

Our MD's are going to start using the medical student notes. The TP guidelines has such a brief blurb that it leaves questions for me. They are trying to develop examples of linking statements. I'm searching the web for examples but haven't found anything yet. I'm assuming that the statement will have to be different than the statement used for residents.

Anyone have a link to examples?
