Hello, I am looking for guidelines on if a medical practice charge for equipment or supplies that were given at no cost to the provider office.

Thank you for the response. The CPT code includes the supply as part of the complete service. 95250 includes professional (office) provided equipment.
Were also looking at 95249 however the equipment is not patient provided. I am afraid that telling the insurance company that the patient has the equipment might have bad repercussions??

I may be overthinking but if we report a patient supplied glucose monitor and then later the patient needs another one Im afraid insurance wii deny because we reported the patient has their own. We should call the carrier I suppose.
 
You are WAY overthinking. A glucose monitor is not a once-in-a-lifetime item. If the insurance has paid for one, then they are aware of it. If they have not paid for one, then they will cover one.
