Does anyone know...

Does the HRA have to be a separate form or can the questions be incorporated into the visit itself. Everything I've seen mentions a form, but I thought I read somewhere that an actual form is not necessary. I can't remember where I read that. I would need a reference cited, please. Thanks to anyone who can help!!
