Does anyone know if medicare will pay for any of the following: 96365, 96366, J7040, J0600, E0845, L1907, L0648 and E0730? Do you know how I could have looked it up on the CMS website?

My understanding is that DME is processed through a separate contractor. Ours goes through Cigna Medicare Services; their website has TONS of info on medical necessity requires, billing requirements, etc. Not sure if there is a different contractor for each state like there is for Part B services, you might have to search for that. But CGS is a good starting point:
