Does anyone have written documentation (we are in Upstate NY) that says the provider billing or Medicare will do the carve out reduction when billing for a PE service and E/M service on the same date. The claims processing manual does not indicate if the reduction needs to be done by the provider billing or if Medicare automatically makes the reduction. Any material would be greatly appreciated.
