Wiki Medicare changes to Modifiers on L1852 and L1845?

Does anyone know if Medicare has changed its modifier requirements for L1852 and L1845? We are a DME manufacturer and had a claim rejected for using the incorrect modifier. We had the correct laterality. Does anyone know if KX is always required as well? Thank you in advance! - Cindy
