According to the LCD, it states a patient "must be insulin-treated with multiple (three or more) daily injections of insulin". What about patients that are using long acting insulin, ie: Lantus, who inject 1-2 times a day? Are they still eligible?
 
Not according to the LCD. That does not mean your doctor cannot open a dialogue with the CAC and see if the LCD can be changed given today’s insulin’s and treatment regimens.
