Wiki Medicare denying all 96372 injection codes!! Please help!

It seems that Medicare has started denying all 96372 codes even when there is a 25 modifier on the E/M. Has anyone else been seeing this or does anybody know what to do?? Thanks in advance :)
 
We are having the same problem. Denial code is CO-16 missing information. Can anyone help on how to get these paid for.
 
We are having same/similar issue, it just started. Medicare is only denying 96372 when billing with J1885. Our typical claim would be 99203-25, 96372 & J1885 (POS 20). We are getting Co-16, M51 & N56 for the denial reasons.

Check to make sure that the NDC is coded with the J1885
 
I called Medicare about this denial for 96372. The rep said it their system is denying in error and they are working on a resolution. Her recommendation was to check in back in a week or two to see if Medicare is going to special batch these claims or if they'll need to be refiled once the error is corrected.
