can someone please verify what it means when a cpt (surgery)on medicares fee schedule has only one fee set? does this mean this procedure can NOT be done on office? there are no LCDs listed for this cpt for my state thank you!
 
Do you mean it does not have both the regular rate and the facility rate? What is the CPT you are looking at?
