Z87.891 is personal history of nicotine dependence. You would not use the personal history diagnosis if the patient is a current smoker. (Z87.891 has an Excludes1 note for current nicotine dependence F17.2-F17.299.)

Before the patient's first lung cancer screening, you also have to counsel the patient at a shared decision-making visit. You can find the G0296 code for that at the MLN link above as well.
